Objective:
As a Support Worker, you will play a crucial role in empowering our clients to live as independently as possible while providing them with high-quality care and support. You will primarily work in a designated service setting but may be asked to assist in other locations as needed.
Key Responsibilities:
1. Client Care:
• Deliver care in accordance with individual support plans
• Monitor, report, and record changes in clients' physical, mental, and emotional health
• Assist clients with daily living activities, including personal hygiene, clothing, laundry, maintaining personal environment, sleep, and activity
• Respect and support clients' customs, individuality, values, sexuality, and spiritual beliefs
2. Social and Emotional Support:
• Encourage clients' engagement in social activities while maintaining professional boundaries
• Establish and maintain professional relationships with staff, other professionals, and agencies
• Assist clients in accessing community-based activities
3. Positive Behaviour Support (PBS) Collaboration:
• Understand the importance of PBS in promoting clients' well-being
• Collaborate with PBS practitioners to create and implement effective support strategies
• Integrate PBS principles into daily support routines
